How to File a Death Claim
We understand the many hardships that one incurs when a loved one passes away, therefore we have made the claims process as simple as possible.

Instructions on Making a Beneficiary Claim
To begin making a claim on a life insurance policy or annuity contract the Beneficiary would need:
- An original Death Certificate (we are able to return it upon request)
- A photocopy of the Beneficiary’s Driver’s License or U.S. Government picture ID
- The original signed Death Claim Form (download from below)
